جدا سازی گونه های کاندیدا از نمونه لاواژ برونکوآلوئولار (BAL) بیماران ریوی در بیمارستان شریعتی تهران

<p> <a name="OLE_LINK11"></a><a name="OLE_LINK10"></a><a name="OLE_LINK9"> <strong>سابقه و هدف</strong> </a><i>: </i><i>شناخت کاندیدیازیس ریوی همواره با مشکلاتی مواجه بوده است. گرچه جداسازی گونههای کاندیدا از نمونه لاواژ برونکوآلوئولار </i><i>(BAL) </i><i>به طور معمول نمایانگر تشخیص قطعی این بیماری نیست، ولی در بیماران مستعد با ایمنی سرکوب شده، جداسازی کاندیدا از ترشحات تنفسی میتواند مفید بوده و ما را به تشخیص و درمان سریع این عفونت قارچی هدایت نماید </i><i>. </i><i /></p><p> <i><strong></strong></i><strong>روش بررسی</strong> <i>: </i><i>در یک مطالعه مقطعی، نمونه لاواژ برونکوآلوئولار 144 بیمار مبتلا به بیماری زمینهای و علایم ریوی مطالعه گردید. از تمامی نمونهها آزمایش مستقیم به عمل آمد و کشت بر روی محیط کروم آگار کاندیدا صورت گرفت. از محیط کورن میل آگار و تست تشخیصی </i><i>API 20C </i><i>جهت تشخیص گونههای کاندیدا استفاده شد </i><i>. </i></p><p> <strong>یافته</strong> <strong>ها:</strong> <i /><i>گونههای کاندیدا از 74 نمونه (4/51 درصد) جدا گردیدند. شایعترین گونه جدا شده، کاندیدا آلبیکنس (7/69 درصد) و سپس به ترتیب کاندیدا گلابراتا (2/20 درصد)، کاندیدا کفایر (6/5 درصد)، کاندیدا کروزهای (2/2 درصد) وکاندیدا تروپیکالیس (2/1 درصد) بودند. هیپرکلونیزاسیون ( </i><i>cfu/ml </i><i>10<sup>3</sup> </i><i>≤ </i><i>) در 10 بیمار دیده شد </i><i>. </i><i /></p><p> <strong>نتیجه </strong> <strong>گیری </strong><i>: </i><i>کاندیدای غیرآلبیکنس (3/30 درصد) در نمونه های تنفسی بیماران این مطالعه شایع بوده و نیازمند روش هـای تشخیصی و پیگیری است. حساسیت کم گونه های غیر آلبیکنس نسبت به داروهای متداول در عفونت های قارچی و هیپرکلونیزاسیون باید در نظر گرفته شود </i><i>. </i></p><p> </p>
<p> <strong> Background: </strong>Definite diagnosis of <i>Candida</i> pneumonia remains a problem in medicine. Although, isolation of <i>Candida</i> spp. from the bronchoalveolar lavage is not diagnostic by itself, it may be helpful in early diagnosis of pulmonary candidiasis in immunocompromised or critically ill patients and prompt antifungal therapy. <strong></strong></p><p> <strong> Materials and Methods: </strong>In a cross- sectional study, bronchoalveolar lavage was obtained from 144 patients with pulmonary diseases and different predisposing factors. All specimens were examined by direct microscopy and inoculated on CHOROM agar candida plates. Corn-meal agar medium and API 20C diagnostic kit also were used for identification of different stains of <i>Candida</i>. </p><p> <strong> Results </strong>: <i>Candida</i> spp. was isolated from 74 (51.3%) specimens. The most common isolated species was <i>Candida albicans</i> with frequency of 69.7%, followed by <i>C.glabrata </i>(20.2%), <i>C.kefyr</i> (5.6%), <i>C.krusei</i> (2.25) and <i>C.tropicalis</i> (1.2%). Hypercolonization (≥10<sup>3</sup> CFU/mL) was found in 10 patients. </p><p> <strong> Conclusion: </strong>This report implies the high frequency (30.3%) of non- albicans <i>Candida</i> spp. isolated from respiratory tract, necessity the application of other diagnostic methods and following up the patients. Less susceptibility of <i>non- albicans Candida</i> spp. to present anti- fungal drugs must be noted and hypercolonization of respiratory tract with <i>Candida</i> spp. should be considered as a initiating factor for invasive candidiasis. </p>
